Candidate in Training Report II - Medical

            ~ Social Security Trace/Validation, Academic Verifications*, Training Verifications, Licensure

               Verifications (limit 2), FACIS/OFAC Query, Multi-state Criminal Database, Driving Record,

               (2) Upper Civil and Criminal County Court Record Search, Federal Court Record Search (Civil,

               Criminal, and Bankruptcy)

Basic Court Investigation

Levels I through III are suggested for candidates that have been in practice for less than 7-10 years

Level I Screening Report – Medical

            ~ Social Security Trace/Validation, Academic Verifications *, Training Verification,

               Licensure Verifications, (2) Upper Civil and Criminal County Court Record Search,

               Federal Court Record Search (Civil, Criminal and Bankruptcy), OIG Medicare/Medicaid

               Sanction Query

Level II Screening Report – Medical

            ~ All services provided in the Level I Screening Report (+) DEA Registration Query, Board

                     Certification Verification, and FACIS / OFAC Query

Level III Screening Report – Medical

            ~ All services provided in the Level II Screening Report (+) GSA Debarment Query, Driving
                      Record History (license number and issuing state required)

Levels IV and V are suggested for Candidates that have been practicing

over 7-10 years

Level IV Screening Report

        ~ Social Security Trace/Validation, Licensure Verifications, FACIS/OFAC Query, Board Certification

            Verification, DEA Registration Verification, Multi-state Criminal Database, National Sex Offender

            Registry Query, Driving Record, (2) Upper Civil and Criminal County Court Record Search, Federal

            Court Record Search (Civil, Criminal and Bankruptcy)

Level V Screening Report – Medical

            ~ All services provided in the Level IV Screening Report (+) 2 Malpractice Claim Histories --
                       company and policy number required and (2) Staff Affiliation Verifications
